Prior to participating in a test, all testers are asked to complete a Confidentiality form. Occassionally, FPI conducts focus groups sessions that are recorded. In these instances, testers would also complete a Video Release form. This is usually done at the test site.
Kids' tests require similar forms. If your child is participating in a test, but you will not be bringing them to the test location, please print out the forms below. Please complete the forms and have your child bring them to the test, and turn in to the hostess at check-in. Completed forms are necessary for your child's participation.
